Observed behavior
A completed review records whether focus enters and stays within the open dialog, whether the close control is reachable, whether Escape dismisses it and where focus returns after closing.
User impact
The review should distinguish intended modal containment from an unreachable close control, unusable dismissal or incorrect focus return.
Recommended next action
Define the dialog's focus entry, visible close control, focus return and exit behavior; test the complete path with keyboard and screen reader output.

Is one finding a full audit?

No. A complete audit requires an agreed product, templates, journeys, states, environments, criteria and reporting scope.

Can the same format support an ACR?

It can inform a scoped evaluation. An ACR requires its own product scope, evidence, criteria, review responsibilities and approved deliverables.
